SEABURY AT HOME – OFFERS SENIORS AN AFFORDABLE SOLUTION AND PREDICTABLE COSTS FOR FUTURE CARE AT HOME
Seabury At Home is the first home-based Life Care program in Connecticut, offering independent and active seniors the option of realistically planning for a future at home, while maintaining their independence with the security of future care at little or no additional cost. Seabury At Home is the result of extensive research, as well as years of painstaking groundwork and a change in legislation pioneered by Seabury.
Seabury At Home is modeled after Seabury, a not-for-profit, interfaith, active life community in Bloomfield, that combines exceptional housing and health care, promoting successful aging and a high quality of life. Founded in 1876, Church Home of Hartford Incorporated (CHHI), d.b.a. Seabury, is an affiliate of the Episcopal Diocese of Connecticut. Seabury At Home represents a significant step in Seabury’s/CHHI’s on-going effort to carry out its primary mission. It is the natural next step to expand its mission to include a continuum of care to people who wish to remain at home.
Numerous studies have shown that 80-90% of all seniors prefer to stay in their own homes as they age. More significant is the fact that many people expect that Medicare and Medicaid are going to cover the costs of long-term care at home. According to the University of Connecticut Health Center's Connecticut Long Term Care Needs Assessment, Executive Summary (2007), “the majority (almost 80%) of respondents express a strong desire to remain in their own home with home care services and supports as necessary.” Conversely, “almost half of those over age 60 (46%) think that Medicare will pay for their care. Clearly, it is not well known by many Connecticut residents that Medicare actually pays very little for long-term nursing home, or home and community-based care. In addition, Medicaid, the anticipated long-term care funding source for over one-fifth of respondents, does not support the vast majority of people in their homes. This creates a dissonance between residents' strong desire to continue living in the community with supports, and how they plan to pay for such care."
Seabury At Home offers the “best of both worlds” for those who want to remain in the comfort of their own home while having all of the benefits of a traditional CCRC. Additionally, Life Care provides the opportunity for Members to pre-pay for services, and protect their financial assets by having predictable health care costs.
Furthermore, Life Care At Home offers a more affordable option to those who may not have the means to move into a Life Care community, and will benefit the state by keeping people off the Medicaid program.
Pricing is determined by the age of the applicant, the younger the age the lower the fees, and includes a one-time membership fee paid at the time a person joins, and monthly fees that start the first day of membership and continue for as long as a person is a member.
Seabury At Home's philosophy is built on a wellness approach, with emphasis on
prevention, which will enable one to maintain their independence for as long as possible.
Upon enrollment, an applicant needs to be relatively healthy with no degenerative disease diagnoses, and independent with no need for immediate services. However, unlike Long Term Care Insurance, benefits start the moment you join, with no waiting periods and continue for a lifetime. Moreover, Members also have the assurance of knowing that a local, well-respected organization will be providing their care when needed, and for as long as necessary.
A personalized approach is the foundation of the Seabury At Home program. A Personal Health Coordinator (PHC) will meet with you to provide a wellness/lifestyle plan, and will be available, via phone, 24 hours a day/7 days a week. Additional Membership services available immediately include Emergency Monitoring & Response System, Annual Home-Safety inspections, Access to Seabury's State-Of-The-Art Fitness Center, Social and Wellness Programs, and Coordination/Referral Services for Home Maintenance, Repairs, etc.
The PHC will be a single point of contact, a professional resource for one’s health care needs, and will arrange for membership services when a member's health declines. These services include Transportation, Home Nursing (skilled care), Home Health Aides (personal care), 24/7 Live-in Companions, Adult Day Care, Coordination of Home Delivered Groceries/Meals, Supportive Technologies, Priority Access to Seabury's Assisted Living, Nursing Home, and Memory Support Center (or another credentialed facility), at little or no additional fee, depending on the selected program.
