SEABURY AT HOME – OFFERS SENIORS AN AFFORDABLE SOLUTION AND PREDICTABLE COSTS FOR FUTURE CARE AT HOME Seabury At Home is the first home-based Life Care program in Connecticut, offering independent and active seniors the option of realistically planning for a future at home, while maintaining their independence with the security of future care at little or no additional cost. Seabury At Home is the result of extensive research, as well as years of painstaking groundwork and a change in legislation pioneered by Seabury. Seabury At Home is modeled after Seabury, a not-for-profit, interfaith, active life community in Bloomfield, that combines exceptional housing and health care, promoting successful aging and a high quality of life. Founded in 1876, Church Home of Hartford Incorporated (CHHI), d.b.a. Seabury, is an affiliate of the Episcopal Diocese of Connecticut. Seabury At Home represents a significant step in Seabury’s/CHHI’s on-going effort to carry out its primary mission. It is the natural next step to expand its mission to include a continuum of care to people who wish to remain at home. Numerous studies have shown that 80-90% of all seniors prefer to stay in their own homes as they age. More significant is the fact that many people expect that Medicare and Medicaid are going to cover the costs of long-term care at home. According to the University of Connecticut Health Center's Connecticut Long Term Care Needs Assessment, Executive Summary (2007),
Seabury At Home offers the “best of both worlds” for those who want to remain in the comfort of their own home while having all of the benefits of a traditional CCRC. Additionally, Life Care provides the opportunity for Members to pre-pay for services, and protect their financial assets by having predictable health care costs.
Furthermore, Life Care At Home offers a more affordable option to those who may not have the means to move into a Life Care community, and will benefit the state by keeping people off the Medicaid program.
Pricing is determined by the age of the applicant, the younger the age the lower the fees, and includes a one-time membership fee paid at the time a person joins, and monthly fees that start the first day of membership and continue for as long as a person is a member.
Seabury At Home's philosophy is built on a wellness approach, with emphasis on prevention, which will enable one to maintain their independence for as long as possible. Upon enrollment, an applicant needs to be relatively healthy with no degenerative disease diagnoses, and independent with no need for immediate services. However, unlike Long Term Care Insurance, benefits start the moment you join, with no waiting periods and continue for a lifetime. Moreover, Members also have the assurance of knowing that a local, well-respected organization will be providing their care when needed, and for as long as necessary.
A personalized approach is the foundation of the Seabury At Home program. A Personal Health Coordinator (PHC) will meet with you to provide a wellness/lifestyle plan, and will be available, via phone, 24 hours a day/7 days a week. Additional membership services available immediately include Emergency Monitoring & Response System, Annual Home-Safety inspections, Access to Seabury's State-Of-The-Art Fitness Center, Social and Wellness Programs, and Coordination/Referral Services for Home Maintenance, Repairs, etc.
The PHC will be a single point of contact, a professional resource for one’s health care needs, and will arrange for membership services when a member's health declines. These services include Transportation, Home Nursing (skilled care), Home Health Aides (personal care), 24/7 Live-in Companions, Adult Day Care, Coordination of Home Delivered Groceries/Meals, Supportive Technologies, Priority Access to Seabury's Assisted Living, Nursing Home, and Memory Support Center (or another credentialed facility), at little or no additional fee, depending on the selected program.
Seabury At Home Incorporated is a subsidiary of Church Home of Hartford Incorporated, d.b.a. Seabury, and supports its mission by bringing affordable, home-based life care to seniors, while enhancing their wellness and quality of life. Seabury At Home is designed for healthy, active adults, 60 and over, who reside in and around Hartford County.
There is no better time than today to take advantage of the many benefits of Seabury At Home. With Seabury At Home, there is no disruption to your everyday life – you need not change a thing. Continue to enjoy life to the fullest in the luxury and privacy of your own home with complete peace of mind.

CANPFA DISTINGUISHED SERVICE AWARD John Mobley was the recipient of the Distinguished Service Award at the recent CANPFA annual meeting held in Westbrook, CT on October 21, 2008. CANPFA, the Connecticut Association of Not for Profit Providers for the Aging, presents this award to an individual whose “dedicated leadership, outstanding service, and personal commitment over the years has furthered the goals of CANPFA and enhanced the quality of life for the elderly we serve.” John was cited for his strong leadership, vision, and commitment regarding elderly services, which have helped guide CANPFA and other organizations serving the senior population to prepare for the future and present needs of the senior population. His involvement as a Board member of CANPFA and as a member of AAHSA, the American Association of Homes and Services for the Aging, House of Delegates resulted in open dialogue and future planning regarding services for the elderly − statewide and nationally. John’s exceptional leadership is perhaps nowhere more evident than in his recent efforts working with our state legislators to pass groundbreaking legislation enabling the creation of the first home-based life care program for seniors in Connecticut – Seabury At Home. In his remarks, he thanked the many compassionate, dedicated, and caring people on our Board for their service, support, and long-term vision. He also praised the contributions of Seabury’s leadership team (many of whom were in attendance) and concluded by thanking the “true heroes that made this award possible – the employees back at Seabury carrying out the day - to - day duties of providing care and services to residents. CHARITABLE GIFT ANNUITIES – A SAVVY ALTERNATIVE DURING TOUGH TIMES IN THE STOCK MARKET Now more than ever, with many investments giving minimal or negative returns, a Seabury Charitable Gift Annuity is a great way to give a philanthropic gift and receive an excellent return on your investment. The minimum age to purchase a Seabury Charitable Gift Annuity is 62. The minimum amount required to fund an annuity is $10,000, but you may create your annuity in any amount over the minimum. This effectively offers you the option of having your money work for you in the following ways:
Why support Seabury through the purchase of a gift annuity?
Church Home of Hartford (CHHI) and the Seabury Charitable Foundation assure that any resident will never have to leave due to lack of funds. In the past 10 years, Seabury Charitable Foundation has provided over $1,000,000 in financial assistance to residents, who can no longer pay for services. By investing in a Seabury Charitable Gift Annuity, you are helping your neighbors, while strengthening your own financial position. It is definitely a win-win situation! If you are interested in more information, Pegeen Sullivan will be happy to provide a personalized proposal with specific figures as they apply to you and the amount of your gift. FIFTH ANNUAL SEABURY CHARITABLE FOUNDATION GOLF TOURNAMENT A huge thank you to all who made the 5th Annual Seabury Charitable Foundation Golf Tournament such a great success. We were able to raise close to $50,000! More than 100 golfers enjoyed a great day of golf at Wintonbury Hills Golf Course in Bloomfield, CT, followed by a fabulous dinner and silent auction. Tournament proceeds benefit Seabury’s health service for the aging. These include respite and long-term care at Seabury Meadows Memory Support Center, Seabury Home Care support for local communities, The Technology Initiative Program, and other services benefiting seniors with limited resources. We look forward to our 6th Annual Seabury Charitable Foundation Golf Tournament next year. Save the date ~ September 15, 2009! SEABURY'S CERTIFIED FITNESS & WELLNESS INSTRUCTORS MAKE A SIGNIFICANT DIFFERENCE IN THE LIVES OF OLDER ADULTS Seabury’s fitness instructors are nationally certified and hold specialty credentials in a variety of disciplines such as corrective exercise, pilates and yoga, aquatics, and integrated balance. Additionally, their background and experience working with older adults has further strengthened the success of the program. Seabury’s fitness instructors are required to stay up to date on the latest in research by attending conferences and workshops. They use the latest technology for evidence-based tracking that enables them to provide a thorough analysis of resident needs and desires. This analysis allows our instructors to develop a custom-made, personalized fitness and wellness program for each participant, as well as each class that they teach.
Seabury has a comprehensive group fitness program, offering classes that address resident needs such as strength, agility, endurance, flexibility, and balance. Some of the current class offerings include strength classes such as Mat Pilates, Core Essentials (a mat class that targets the abdominal, lower back, pelvic floor and hip muscles) and Yogalates (Yoga and Pilates combined). For agility and balance, our fitness instructors offer Dancercise, Step (these participants step up, over, and around a four-inch bench while keeping in time with the music), and Equilibrium (a balance class that focuses on five critical areas of balance: postural strategies, gait enhancement, multisensory control, center of gravity control, and basic fitness). They also offer aquatic programming: Yoqua (Yoga in the water) and Aqualibrium, which has the same goals as the aforementioned Equilibrium, but in the forgiving medium of the water. It is evident in the variety of classes developed for older adults that Seabury’s Fitness & Wellness Center has taken instruction to a completely new level. The fitness instructors take great care and time in developing their fitness plan for the group/class curriculum. Reports are developed based on each participant’s individual assessments. These reports use graphs and pie charts to show areas of fitness that the group/class have in common for being at-risk or below average in losing functional mobility. The fitness instructors then take that data and develop concrete goals and plans of action for each class. An example would be:
Dancercise Class Held: M-W-F, 9:15-10 AM
Action: Concentrate on power moves during choreography and significant lower body exercises during strength portion of class
Goal: to bring 10% of the class out of at risk/below average category
2. Lower Body Flexibility - 25% are at risk
Action: include dynamic stretching in the choreography and static stretching in both the warm-up and cool-down
Goal: to bring 5% of the class out of at risk category
3. Upper Body Flexibility-18.18% are at risk, and 18.18% are below average for a total of 36.36% needing improvement.
Action: include upper body movement during choreography to warm upper body muscles for more intensive stretching during cool-down
Goal: To bring 10% of the class out of at risk/below average category. Furthermore, they discuss these goals with each class, and then re-evaluate the program at the end of each quarter by comparing and contrasting the assessment data.
Seabury takes particular pride in its quality of its fitness and wellness program development. Our staff is completely vested in the well-being of its participants and committed to providing the best the field has to offer. The proof of this dedication is seen in the results from our tracking software. In fact, the Fitness and Wellness Center is seeing such a success in keeping participants healthy that everyone is living a more healthy lifestyle than they were when they were younger! CIRCLE OF WELLNESS: PULL OUT YOUR PILLOW-NAPS ARE GOOD FOR YOU That’s right— there is now proof that catching a few zzz's in the afternoon can be beneficial to your health.
A recent study by the Institute of Medicine in Washington, D.C., estimates that 50 to 70 million Americans suffer from chronic sleeping disorders, "hindering daily functioning and adversely affecting health and longevity."
Obviously, if you lay down for about 4 hours in the evening, and wake up at 10p.m. expecting to go to bed at 11:00p.m., you’ll be in for a long night. While each individual is different, it is suggested that 20 - 90 minute naps are optimal and do not affect nighttime sleep. The best time to nap is 1-3pm, when the sun is at its peak. (Ever hear of the siesta in Spain or Mexico?) Consider this, humans are the only mammals that sleep in 1 phase per day. All other animals sleep in multi-phases, just watch your dog or cat everyday, they love to take naps! www.sleepfoundation.org
